Rehabilitation Services & Skilled Nursing Care Facility in Wichita, KS
Short-term skilled nursing helps patients recover following an acute illness or injury, allowing more time to heal post-hospital and greater opportunities to be safe and successful after they return home. A patient may require a stay at a skilled nursing center following a surgery, accident, fall, stroke, heart attack, or other significant medical events.
Skilled nursing facilities (SNFs) are different from long-term care centers or assisted living communities. They are a temporary place to rehab and recover. The typical skilled nursing facility stay is 14–21 days.
Planning Ahead…
After goals set by the patient’s care team have been met, the patient can transition home. If the patient’s change in medical condition has affected their ability to return home safely, decisions can be made to move to a higher level of care—for example, from Independent Living to Assisted Living, or Assisted Living to Long-Term Care.
Our team of social workers works with patients and family members to develop a plan for a return home or the next level of living. They collaborate with the care team to ensure all future needs are met and patients are successful in their continued journey toward independence.
