Skilled Nursing Post-Acute Care Center at Catholic Care Center
The final phase of Catholic Care Center’s repositioning and renovation project was finished in early January with the completion of the Post-Acute Care Center (PACC) renovation. This project was launched in June of 2022 and included the renovation of several areas of long-term care and the creation of the St. Veronica Senior Behavioral Health Hospital. The PACC unit was part of the original Catholic Care Center built in 1990 and previously consisted of 28 beds, a combination of private and semi-private rooms. Over the years the area had seen some cosmetic updates, but it was in need of a more major renovation to better serve the highly acute, complex patients that our ministry is called upon to serve.
During the first week of January, patients and associates were welcomed into the new PACC unit. It was exciting to see the reaction of our dedicated associates who were overjoyed at having this new, modern space to better care for their patients. Patients and their families were thrilled to see the new area and have a more comfortable and enjoyable space to stay during their recovery journey.
The new PACC features private rooms with private showers. The rooms are centered around a central nurses’ station and beautiful dining and activity spaces. Residents have convenient access to St. Elizabeth Chapel for daily Catholic Mass and to the therapy gym. This new space allows us to combine the outstanding care that our skilled nursing program is known for, with the benefits of a newly constructed, and more easily accessible unit. The new PACC can accommodate up to 43 patients at a time with the extension into the former Whispering Brooks neighborhood and so far interest has been strong with a sharp increase in hospital referrals and inquiries. We look forward to capitalizing on this opportunity to better serve patients, and help them get back to living the life they love!

After goals set by the patient’s care team have been met, the patient can transition home. If the patient’s change in medical condition has affected their ability to return home safely, decisions can be made to move to a higher level of care—for example, from Independent Living to Assisted Living, or Assisted Living to Long-Term Care.
Our team of social workers works with patients and family members to develop a plan for a return home or the next level of living. They collaborate with the care team to ensure all future needs are met and patients are successful in their continued journey toward independence.
